Non-Discrimination

We do not discriminate in the provision of services to an individual for inability to pay; nor because payment for those services would be made under Medicare, Medicaid or the Children’s Health Insurance Program (CHIP); We do not discriminate based upon race, color, sex, national origin, disability, religion, or sexual orientation.

Forms and Letters

The Therapist, PLC does not provide disability letters, companion pet letters, or letters regarding your ability to work, or any letters that would inform providers of your mental health history. You must discuss these needs with your medical or primary care provider.

Cash or Self-Pay Discount

Clients who choose to pay out-of-pocket (self-pay) at the time of service are eligible for a 5% discount off the stated fee for CPT-coded services. This discount applies only when payment is made in full on the date of the session and does not apply to insurance claims, late payments, or bundled service packages.

Life Coaching Services Fees

Life coaching is not covered by insurance. Sessions $90–$250. Packages $100–$1500/month include various coaching features. 10% discount for two-month package.

Sliding Scale

Reduced rates and Sliding Fee scale is available upon request. You can download the forms and submit completed forms for income verification. Services will be provided regardless of inability to pay. Fees Insurance Accepted

Blue Cross Blue Shield MN, Health Partners, MN Medical Assistance, South Health Country Alliance, U-Care, United Healthcare, Aetna, Medica, Medicare, Some EAP, Out-of-Network and Private pay.
